New Type Rejection

Hi All,

I submitted a new type form last night which was rejected. On the email, it shows I submitted a blank model despite me entering the model in the custom field.

Where did I go wrong for this not to appear properly for the dB team? Also, there was an unsubscribe link in the email that I mistook for a link to the form. Can I reverse this unsubscription? Not sure what I've unsubbed from in honesty!
