    reports now coming in that the operator was Paklook Air, there is always confusion in the initial hours after a crash - for example the major media did not report on the recent 737 Resolute crash for hours and hours because of the confusion with the military simulation that was in progress

        What is surprising is that there have been 6 fatal part 135 plane accidents in the last year in the US excluding this one causing 12 fatalities (6 pax, 6 crew).
        Causes, flight phase:
        1. Decent below mda in fog, approach
        2. Decent below mda in fog, go around
        3. Engine failure, initial climb
        4. Fire, cruise
        5. VFR flight into IMC, maneuvering en-route
        6. Midair collision during a too close formation flight, cruise

          Two passengers aboard were rescued but sadly the pilot perished. They were 5 minutes from Kodiak at the time of the incident at 7:55 in the evening.
