Aviation related was when I was doing my first solo I forgot to take the chocks out from both mains.
Non aviation related has to go to hockey. Nothing specific, but I have let in some really bad goals in my life. I'm one of those that can stop the most difficult shot ever, but if someone dumps it in from the red line the chances of it going in are pretty high.
